Aim:
Description of initiative: One such initiative was introduction of Expert Clients (patients on ART) to help Medical health professionals in preparing, monitoring and supporting patients in comprehensive care and positive living in the context of clinical care of HIV/AIDS specifically when on ARVs.
 
Method / Issue:
Issue: AIDS medicines have been portrayed and perceived as a complex therapy the world over hence creating a kind of specialness around them. In Ugandas resource limited settings, such specialness created a necessity for initiative and significant re-organization of health service delivery. Methodology: An in-depth study using a qualitative, socio-anthropological approach (observations and interviews) was conducted in 2 model centers i.e. Kasana-luweero health center IV and Kitovu mobile HealthCare in Masaka district between February 2006 and November 2006 to learn from district level transformations in providing care in the advent of ART.
 
Results / Comments:
Findings/Lessons Learned: 1. With the specialness created around ART, the concept of medicine as usual is dying out. 2. Whereas the practice of medicine has been historically contingent today there seems to be several control mechanisms. 3. A new question of moral custodianship of medical knowledge has arisen 4. New tensions have arisen between providers and patients with regard to loyalties, boundaries, acceptance and legitimacy
