Aim:
The initiation of long term therapy among vulnerable women in Africa raises concern because of their marginalized conditions with very little social and familial support. As part of the ANRS 1285 trial, we evaluated the predictive factors associated with poor compliance to 3 months of daily Valacyclovir or placebo.

Method / Issue:
, and VAN DE PERRE Philippe The trial included HIV-1/HSV-2 co-infected individuals not eligible for HAART including vulnerable women. Before randomization, each participant went trough interviews with psychologists who performed in-depth interviews focused on the different factors related to regular intake and social management of long term therapy, that have been previously identified in a pilot study. All participants benefited from scheduled interview to support compliance during the course of the trial. We performed a qualitative analysis to identify the factors associated to compliance issues.

Results / Comments:
Forty trial participants have been included in this study. The qualitative analysis showed that the main factor associated with compliance issues was disease denial. Furthermore, younger women showed less commitment to regular drug intake, reported higher mobility, greater social activities which may hamper drug adherence . Beside disease denial, no psychological status seemed to influence drug adherence.

Discussion:
This study showed that vulnerable women can achieve a high level of drug compliance. The initial identification of factors that may alter drug adherence enable to adapt and personalise the compliance support during treatment.
