Aim:
The AIDS epidemic is affecting women in increasing numbers, especially in places where heterosexual sex is a dominant mode of HIV transmission, as in Subsaharan Africa and the Carribean. Transactionnal sex, as a response to poverty, is only one of the causes of this feminisation of the epidemic : a large proportion of new infections occur now within the couple, through the sexual transmission of HIV between regular heterosexual partners. We will present in this talk how the conjugal relationship can be at risk for the HIV infection. From recent research results in various contexts, we will discuss why sexual transmission, vertical transmission and the pressure for procreation are to be taken into account all together in a heterosexual conjugal relationship, in a gendered approach. The current tracks to prevent HIV transmission in such regular relationships will be explored : better integration of HIV/Aids prevention in reproductive health services, conjugal counselling, new prevention tools, etc... In particular, we will question how the development of PMTCT programs at a large scale may bring new elements in HIV prevention and access to care at a conjugal level.
