Aim:
Romania has a large group of people living with HIV/AIDS (PLHIV) who are long-time survivors nosocomially infected during the communist regime. Officially, antiretroviral treatment (ART) is guaranteed and all people in need receive free ART, ensuring high treatment coverage at 81%. Maintaining treatment adherence is one of the main challenges for both PLHIV and clinicians, since most people living with HIV are on treatment for more than 10 years and already experienced several switches.
 
Method / Issue:
A community-based research was carried out between 2010 and 2012 (N= 300) in three large cities. Self-reported treatment interruptions resulting either by choice or accidentally were considered as individual factors of poor adherence, while the discontinuous availability from medical facilities a structural factor.
 
Results / Comments:
Concerning individual factors, 276 PLHIV (92% of total sample) declare taking ART and 77 (27.9%) reported no interruptions as a result of forgetting and as a result of own choice in the previous 12 months. However, 46 out of 77 (59.7%) reported ART being unavailable at medical facilities at least once in the last year.
 
Discussion:
While both clinicians and PLHIV address individual factors (e.g. by treatment adherence counseling, text messaging service), structural factors (e.g. discontinuities in treatment supplies) undermine efforts made by clinicians and PLHIV.
