Aim:
In Siaya and Bondo counties HIV prevalence is among the highest in Kenya. More females than males are accessing services referral is weak and knowledge about HIV and safe sex low. Through a mapping exercise in 2012 it became clear that most NGO’s are targeting girls. With a consortium of partners an intervention study in Bangladesh and Kenya started applying a motivational interviewing intervention (MI+) consisting of different trainings by the Public Health Service Amsterdam (GGD) the Netherlands and setting up a support system of master trainers and representatives. Nurses, AIDS coordinators, teachers, peer educators and counselors were trained in MI with the aim to make their services more (male) adolescent friendly and improve the communication with young males.
 
Method / Issue:
At present the intervention is still running with an after study with 1200 young males and key informants planned for at the end of this year. During the intervention two qualitative trajectories were implemented. In June and December 2014 in-depth interviews and FGDs were held with around the same group of 60 young males, nurses, teachers and peer educators.
 
Results / Comments:
One of the main findings is that the intervention has led to successful networking and linkages between hospital, youth clubs and schools. In the HMIS we found an increased number of males coming to the facilities 5- 15 per month. There was increased positive role modelling among the participants of MI+ session and utilization of the index clients to mobilize fellow youth. Increased trust from the parents in the community allowing their children to associate with the groups reaching other youth by applying MI skills.
 
Discussion:
During the trajectory it appeared that young males are actually often going for HIV testing but not in nearby clinics through this project they have made aware that they should also protect themselves better and have a right to access the services in their surroundings .The service providers underlined that now they are using a less paternalistic approach even sending young people away when they come for services and do apply this also on other clients with success. They even claimed that more clients come and ask for their services as they are seen as better equipped technically and personally. The qualitative trajectory shows that promising steps are being made towards safer sexual behavior among young men contributing to the reduction of HIV. However, upscaling and creating a more enabling environment - meaning training more actors - is needed. The project so far shows that the introduction of a motivational intervention has increased not only awareness and access of young males to services but has contributed to higher quality of care and better linkages and integration of services. Higher numbers are needed and upscaling seems a natural next step in HIV and STI prevention.
