Aim:
- Evaluate the direct costs of HIV-management which are borne by patient according to the type of health service and patient; - Classify the costs according to the health services; - Stand out the costs which are the heaviest for the PLWHA according to the nature of health service and patient.

Method / Issue:
Data were collected from august to December 2006 in 30 public, private, associative and confessional health services which have had activities in HIV-management in Burkina Faso. In each service, the head of the service, 5 persons of the staff, 20 patients under ART and 10 without ART were interviewed. Epidata and SPSS 11 were used for data processing.

Results / Comments:
Different costs are borne by patient according to each type of health service. In the public, patients pay for blood tests, transport and ART (cost for ART is 9$US per month). The major cost for many patients in associative and confessional service is transport (average cost is 0.9$US). In private, patients pay for all. About 64 per cent of patient under ART and more than 50 per cent of patient without ART said that costs of HIV management are a burden. The costs which constitute difficulties for patients are medicines against opportunistic infections, blood tests and transport.

Discussion:
Direct costs of management of HIV draw the social economic profile of patient of different health services. The challenge for the authorities is to reduce the costs of management of HIV for the PLWHA if they cant afford the gratuitousness.
