Aim:
HIV services are developing approaches to equip vertically infected young people with skills to manage the challenges of transitioning from adolescence into adulthood. Interventions have often focused on supporting young people through adolescence whilst neglecting transition into adulthood and beyond. Skills required to successfully manage HIV in adulthood include increased responsibility of managing healthcare, ability to negotiate sexual relationships, transitioning from paediatric to adult clinics and entering the workplace and further education. The London (UK) based Looking Forward Project has been providing a transitional service for vertically infected young people for the past ten years. It is an internationally published evidence-based model with successful outcomes. This multi-disciplinary (nursing, psychology, medics, pharmacists) project uses a group work approach, aiming to equip young people aged 12-24 with the emotional, psychological and behavioural skills necessary to face the challenges of living with HIV in adulthood.
 
Method / Issue:
Format: Presentation of Looking Forward Model with emphasis on sharing concrete aspects of content, structure, approaches/exercises, evaluation tools and key learning points; Experiential learning in relation to exercises undertaken; Group work and discussion identifying local service needs and project adaptations. Materials: Each delegate will be given a Looking Forward Model guide, including programme outlines, example exercises and activities, evaluation tools, evaluation data and published articles.
 
Results / Comments:
Learning objectives for delegates will include: Understanding of a variety of transition models; Beyond early adulthood: what next? A focus on supporting young people in adulthood as well as adolescence; Strategies to promote ?buy-in? and commitment from multi-disciplinary services and stakeholders; Promoting the intervention to parents and families and strategies to address their concerns (e.g. discussions about sex, HIV in families); Understanding of the Looking Forward Model, from philosophical underpinnings to operationalization of the model on a practical basis (a ?how to? guide); How to identify the transition needs of young people within their own service context and support in adapting the Looking Forward Model to suit local needs/constraints; Strategies to build sustainability of the intervention.
