Aim:
Given the high prevalence of HIV and STIs amongst MSM it is important to ensure successful delivery of high-quality programmes and to train health facilities to deliver competent and prejudice – free services. A partnership between the SA DOH and Health4Men has developed a unique project that strengthens health and community systems to improve health outcomes for MSM/WSW and LGBT.
 
Method / Issue:
The Health4Men model includes: • Rapid assessment engaging the key population in the initiation of the project. • Establishing partnerships with DOH and LGBTI community organisation. • Health facility sensitization training to address sexuality, attitudes and perceptions of MSM/WSW. • Biomedical skills development ensure that medical staff are able to take a comprehensive sexual history and effectively diagnose and treat MSM/WSW. • Mentoring sessions are provided to ensure competency. • IEC materials, condoms and lubricant are supplied to clinics with supportive technical assistance. • Outreach activities to ensure MSM utilise competent sites. • Follow-up training to ensure ongoing site competency
 
Results / Comments:
4200 clinic staff trained to provide medical and psychosocial care. 40 competent sites have been developed across four targeted provinces. Key lessons: • Decentralized TA provides a context specific approach responsive to a diverse community. • Synergies in improved services and increased community support leads to sustained gains in reach and quality of services. • Health4Men/DOH partnership provides a model for sustainability. • Creation of safe and trusted environment improves individual/collective self-efficacy. • Improved efficacy leads to gains in social capital and health. • Attitudinal and belief changes creates revises stigmatized identities and leads to more pleasurable and responsible sexual practices. • Creation of a trusted healthcare environment contributes to a Development and Human Rights agenda.
 
Discussion:
: Best practices need to be developed through consensus with the MSM/WSW community. This will ensure that there are standardised approaches to implementing HIV prevention interventions and measuring programme quality. New challenges will be to focus on additional high-risk male populations e.g. refugees, prisoners, commercial sex workers and intravenous drug users.
