Abstract #6  -  Prevention of vertical HIV transmission is a core component of HIV prevention in children: the Rizhny Novgorod experience

  Aim:
Vertical transmission is the predominant mode of HIV transmission in children and thus its prevention significantly decreases the HIV incidence in children. The registered number of people living with HIV/AIDS in the Nizhny Novgorod area has reached 6,440 (01.01.2009). Women comprise 44.7% among all the people infected. 495 children have been born to the HIV positive mothers in the Nizhny Novgorod area since the epidemic started. 100 children were born in 2006 (preventive measures complete for 93.0% mother-child cases); 118 children were born in 2007 (preventive measures complete for 95.8% mother-child cases). Our objective was to study the efficacy of HIV mother-to-child transmission preventive measures (MCTP).
 
  Method / Issue:
: It is known that we can use ART regimens for HIV vertical transmission prevention during 3 periods: prevention during the pregnancy, just in childbirth, ART for newborns. Using HIV prevention with ART during all these 3 periods seems to be more efficient. Information about the MTCP and the use of ART regimens during different periods of prevention in 218 children born to HIV positive women in the area in 2006-2007 was studied.
 
  Results / Comments:
The MTCP through antiretroviral regimens were used in 88.3% of all cases studied. According to our studies the evidence that children are HIV infected when we provide ART regimen during all 3 periods was 0.6%, when we provided preventive ART regimens during 2 of 3 possible periods of our preventive programme the evidence was 6.3%. Children had evidence to be infected with HIV from their mother in 35.0% when neither they no their mothers receive ART prevention (p=0.005).
 
  Discussion:
The best results were obtained when the ART regimens were used during all the three periods of MTCP performed – in pregnancy, just in childbirth and for newborns. It reduces significantly the evidence of vertical transmission of HIV.
